If Do Ji Ui had a wishlist for his military service assignment, there was one thing he definitely didn’t want: an island posting.
Unfortunately, that’s exactly where he ends up.
Episode 1 of Doctor on the Edge follows the plastic surgeon’s arrival on remote Pyeongdong Island, where almost everything that can go wrong seems to happen within a single day. Between strange hallucinations, an unexpected medical emergency, and a growing mystery surrounding his past, Ji Ui’s new life gets off to a rough start.

Doctor on the Edge Episode 1 Recap
The story begins during a military training exercise.
A pair of soldiers fall during a wall-climbing drill, leaving one with a dislocated shoulder and the other with severe cuts. Do Ji Ui quickly steps in to assist. He explains that he is serving as a public health doctor, a role that allows him to work in medically underserved areas instead of completing regular military service.
At the medical center, Ji Ui treats the wounded soldier’s injuries. His careful stitching earns praise from those around him, and the patient leaves with a newfound respect for public health doctors.
Afterward, the soldiers chat about future assignments. Ji Ui says he would rather be stationed anywhere than on an island. One soldier responds with a warning, saying islands come with three dangers: accidents, people, and love.
That comment turns out to be more meaningful than it first appears.
Later, Ji Ui arrives at the ferry terminal for his trip to Pyeongdong Island. Before boarding, he takes out some medication. A doctor friend calls and advises him not to continue taking the pills. During the conversation, the topic of Hwayeong also comes up.
Nearby, a young woman is speaking on the phone. Coincidentally, parts of her conversation seem to match what Ji Ui is discussing. She casually remarks that scars can be fixed with money and demands compensation from the person she’s talking to.
Soon afterward, both passengers board the ferry.
The journey is anything but peaceful.
Ji Ui struggles to rest and appears mentally distracted throughout the trip. Then he suddenly witnesses something shocking: the woman from earlier appears to jump into the sea.
He immediately dives in after her.
The problem is that none of it actually happened.
The incident was only a hallucination.
When the story shifts to the island clinic, the staff are already talking about the arrival of a new doctor. Ji Ui wakes up there, confused and trying to understand what happened.
The head nurse introduces him to the facility and starts showing him around.
His first introduction is to Dr. Yong Jucheon from the Korean Medicine department. Jucheon greets him enthusiastically and immediately wants to add him to several island community group chats.
Ji Ui’s office turns out to be the largest examination room in the building, but any excitement quickly disappears once he inspects the equipment.
Most of it is outdated.
The head nurse explains that replacement requests are submitted every year without success. She also tells him that many of the clinic’s patients simply live with long-term illnesses because resources are limited.
Ji Ui’s first request is simple: a new blood pressure monitor.
Things don’t improve when he sees his living quarters.
A centipede falls directly onto his face, sending him into a panic. The nurses hand him bug spray, and he responds by spraying practically every corner of the room.
Back at the clinic, a discussion about home visits takes place. Ji Ui makes it clear that public health doctors are not obligated to continue such visits and refuses to take on additional ones.
While unpacking later, he notices something unexpected.
The suitcase in his room belongs to someone else.
Looking for answers, Ji Ui visits the village hall and reports the mix-up to the village chief. He also asks whether there has been any accident involving the ferry.
The chief insists there has not. According to him, the ferry has operated safely for twenty years.
He then suggests the luggage may belong to Grandmother Mija’s granddaughter and reveals that the same young woman was responsible for carrying Ji Ui after he lost consciousness.
With only directions to a house with a red roof, Ji Ui begins searching.
Before long, he hears cries for help.
It’s the woman from the ferry.
Seeing a dog running behind her, Ji Ui immediately runs away himself. Moments later, it becomes clear the dog is harmless and simply wants attention.
The woman is eventually identified as Ha Ri.
The suitcase exchange follows shortly afterward.
As Ha Ri checks her luggage, Ji Ui notices several medical supplies inside. A centipede suddenly crawls out, but unlike him, she remains completely calm.
She also returns his medication and warns him not to take it because it doesn’t suit him.
The conversation soon turns toward the ferry incident.
According to Ha Ri, she never jumped into the water at all.
Instead, she had been speaking with Grandmother Mija when her scarf blew away. For reasons she doesn’t understand, Ji Ui suddenly attempted to jump into the ocean. To stop him, she hit him on the head and knocked him unconscious.
I have to admit, this revelation caught me off guard.
Ji Ui doesn’t fully remember the event himself, making the situation even stranger.
Elsewhere, Ha Ri and Grandmother Mija spend time preparing a memorial meal for Ha Ri’s parents.
Back at the clinic, Ji Ui finds himself in another awkward situation when he accidentally encounters a naked man in the bathroom.
The man casually introduces himself as Hyeon Chiyeon.
Like everyone else, Chiyeon asks about the ferry incident. Ji Ui continues to insist that his memories of the event are unclear.
While unpacking later, several photographs fall from his belongings. The pictures show Ji Ui alongside a man and a woman wearing hospital scrubs.
The meaning of those photographs remains unknown for now.
As evening approaches, Jucheon invites Ji Ui to dinner with the staff.
The doctors head to the island’s only restaurant. The owner turns out to be the same woman associated with the dog from earlier.
During dinner, the conversation shifts toward medical careers.
Ji Ui reveals that he specializes in plastic surgery.
Chiyeon admits he has not selected a specialty yet and openly criticizes plastic surgeons, describing them as people interested mainly in money.
The comment leaves Ji Ui visibly stunned.
The meal itself isn’t much better.
Ji Ui clearly struggles with the food, but since the restaurant owner is waiting nearby for his opinion, he pretends to enjoy it.
The uncomfortable dinner is interrupted by an urgent call from the health center.
A patient needs help immediately.
Ji Ui volunteers to return and discovers Village Chief Park sitting outside the clinic.
Park complains of indigestion and asks for medication.
Ji Ui isn’t convinced.
After examining him, he realizes the symptoms point to a heart attack rather than a digestive issue.
Once he administers NTG and sees immediate improvement, his suspicion is confirmed.
Even after hearing the diagnosis, Chief Park refuses to go to a larger hospital.
Ji Ui repeatedly warns him about the danger.
While emergency transportation is being arranged, Park quietly leaves.
The situation turns critical when he collapses while lifting a heavy box near his boat.
Ji Ui catches up and performs CPR until a helicopter arrives.
Both men are transported to Asong Hospital, where Park is rushed into emergency care.
Ji Ui, meanwhile, is stranded on the mainland.
The final ferry has already departed.
Following advice from the clinic staff, he rents a motel room for the night. Although he buys ramen, exhaustion wins and he falls asleep without eating.
The next morning brings another problem.
The clinic is overflowing with patients, and Miss Hwang reminds him that failing to return could lead to disciplinary action.
A second warning could prevent him from transferring elsewhere, effectively keeping him on the island throughout his service period.
Panicking, Ji Ui rushes back.
During the trip, he experiences disturbing flashes of past events.
Just as he appears overwhelmed, Ha Ri places headphones over his ears.
The gesture calms him immediately.
Standing beside her at the ferry terminal, Ji Ui remembers the warning he received before arriving on the island.
Avoid accidents.
Avoid people.
Avoid love.
Less than a day after arriving, he has already run into all three.
That’s where Episode 1 ends.
Episode 1 Review
I thought the premiere balanced humor, mystery, and medical drama surprisingly well.
The strongest part of the episode wasn’t the medical case itself but Ji Ui’s uneasy adjustment to island life. Nearly every scene reinforces how far removed he is from his comfort zone, whether he’s dealing with bugs, unfamiliar residents, or situations he can’t fully explain.
The biggest mystery right now revolves around Ji Ui’s mental state. The hallucination on the ferry, his medication, and the brief flashes from his past all suggest there’s a deeper story waiting to be uncovered.
Ha Ri also stands out immediately. Her interactions with Ji Ui are entertaining, but there are hints that she’ll play a much larger role in helping him confront whatever he’s been trying to avoid.
By the end of the episode, I found myself most curious about Ji Ui’s past and the people shown in those photographs. The island already feels packed with stories waiting to unfold, making this a solid opening chapter for the series.
